Geburtsdatum: Tag/Monat/Jahr

Notfallkontakt: Telefonnummern oder Adressen von Angehörigen/ Freunden, die im Notfall kontaktiert werden sollen

Dauerhafte Medikamenteneinnahme:
▶ Name des Medikaments im Feld „Medikament“ notieren
(Dosierungen oder Häufigkeiten sind nicht notwendig)
▶ Sollten mehrere Medikamente aus unten angeführten Gruppen dauerhaft eingenommen werden, bitte entsprechend dieser Reihenfolge in das Feld auf dem Armband auflisten:

1. Blutverdünner

2. Inhalationsmedikamente (z. B. Asthmasprays)

3. Schmerzmittel / Fiebermittel

4. Diabetikermedikamente

5. Epilepsiemedikamente

6. Blutdruckmedikamente

7. Antibiotika / Kortikosteroide

8. Psychopharmaka

Allergien: z. B. gegen Medikamente wie Penicillin, Jod, Kontrastmittel, Lokalanästhetika, Narkosemittel, Soja, Pflaster

Freie Zeile: Platz für wichtige Erkrankungen, wie z. B. Herzfehler, Blutkrankheit, Frühgeburt/SSW bei Entbindung, Asthma, Maligne Hyperthermie, Herzrhythmusstörungen …

② Pfeile: nutzbar, um auf Platzierung des Aufklebers in unmittelbarer Nähe hinzuweisen

③ Runder EmBraceKit-Aufkleber: allgemeine Information, dass ein Notfallarmband im Fahrzeug vorhanden ist (für den Aufkleber daher eine gut sichtbare Stelle innerhalb des Fahrzeuges auswählen!)

Emergency contact: Phone numbers or addresses of relatives/friends who should be contacted in case of emergency

Long-term medication use:
▶ Write the name of the medication in the “Medication” field
(Dosages or frequency are not necessary)
▶ If several medications from the groups listed below are taken long-term, please list them in the wristband field in the following order:

1. Blood thinners

2. Inhalation medications (e.g., asthma sprays)

3. Pain relievers / fever reducers

4. Diabetes medications

5. Epilepsy medications

6. Blood pressure medications

7. Antibiotics / corticosteroids

8. Psychotropic medications

Allergies: e.g., to medications such as penicillin, iodine, contrast agents, local anesthetics, anesthetics, soy, adhesive bandages

Free line: Space for important medical conditions such as heart defects, blood disorders, premature birth/pregnancy week during delivery, asthma, malignant hyperthermia, cardiac arrhythmias …

② Arrows: can be used to indicate the placement of the sticker in the immediate vicinity

③ Round EmBraceKit sticker: general information that an emergency wristband is present in the vehicle (choose a clearly visible place inside the vehicle for the sticker!)
